Preparing for your first visit

Scheduling an appointment with one of our professionals is very easy - call us at (804) 320-7881, choose option 1. 

 

Please be prepared to pay either your insurance co-payment or the amount stated by your intake coordinator.  If you are a self-pay patient, please be prepared to pay the amount stated by our intake coordinator

When you arrive for your first visit, you will be required to fill out several forms. You may download some of these forms or have them mailed to you to be filled out before your visit - see our patient forms page for more information. While it is not essential that these forms be completed before you come for your visit, doing so will make it easier and quicker for your session to get underway. 

Bring your completed forms, as well as any medications or herbal supplements that you are currently taking, with you on your first visit. We ask that you arrive 20 minutes before your scheduled appointment so that you can complete any other paperwork which may not be available from our web site.
