Tucker Psychiatric Clinic has a number of forms to be filled out for the first-time visit.
You may fill out the forms while in our waiting room, or speed the process by requesting the
forms be sent to you by mail, and filling them out ahead of time at home.
To request the forms by mail, contact us by phone or mail, and ask for the forms
needed for the first-time visit. Please remember to bring them with you when
you arrive. Thank-you.
